Return material authorization

Before returning any material to Melexis, we kindly ask you to address your return request via the below form. After submitting the form, please verify that you have received an email confirmation. Once your request has been authorized, you will receive a unique CRC number which must be used in any communication as well as in your shipment.

Submission Requirements: Failure Analysis (FA) Reports

To ensure data integrity and a precise root cause investigation, please adhere to the following submission guidelines:

• Quantity: Submit one entry per lot and per failure.
• Documentation: Each entry must include a standalone FA report written in English.
• Preparation: The IC must be removed from the PCB/module prior to submission.
• Technical Data: Reports must contain all performed test results, including data plots and ABA swap test outcomes.

This level of detail is required to accurately categorize failure modes and expedite the identification of the root cause. Failure to provide a separate entry per lot or including non-English documentation may result in processing delays. The information provided in this form will be protected by any applicable confidentiality terms in the terms of sale for the Melexis products and/or by any NDA between Melexis and your company.

Returned shipments without a unique CRC number can be refused and returned to your plant. Administrative costs for handling unauthorized returns cannot be claimed.* Melexis has the right to reject the RMA request if the IC is in a damaged condition that is preventing further investigations. (e.g. broken IC, missing leads, burnt marks, burnt IC...)* To achieve an efficient investigation, it is important to provide the part on IC-level. When the part is provided on module level (potted, overmolded…) we cannot guarantee a successful extraction from our side. We have the right to refuse these parts. If we accept the parts, Melexis is not liable for damages incurred on the part due to extraction.* Returned material needs to be properly packed considering ESD risk and environmental risk. For obvious safety reasons, please highlight if returned material is potentially contaminated with hazardous material. In such a case, please provide the relevant MSDS documents along with the returned material.* This RMA is under no circumstances an acknowledgment of Melexis' responsibility or liability for the alleged defect.*
